All these activities reflect well on ICAO, its officers and secretariat. In common with other organisations, ICAO has suffered when member states, including the USA, have not paid their dues or have paid them in arrears.

One of the best known is the disaster that resulted from the collision of two Boeing 747 aircraft on the runway at Teneriffe on 27 March 1977. Fog was a major factor in the accident, and the pilot of one aircraft started his take-off run while the runway was still obstructed, because he believed that he had been cleared for take-off although a transcript of the radio transmissions shows that he had not. He also used the misleading phrase 'We are now at takeoff', causing the air traffic controller to believe that the airplane was at the take-off point, not taking-off.
