Alemanno and E. Egarter Vigl In our anatomical description of the innervation of the supraclavicular region, we cannot neglect to make some mention, however brief, of the cervical plexus.
A second efferent group forms the so-called vertebral nerve or, better, vertebral plexus, following the artery of the same name and insinuating itself with its branches in the foramina of the transverse processes of the cervical vertebrae. A third group, with an inferomedial course, forms the inferior cardiac nerve (cardioaccelerator) after connecting via an anastomosis to the middle cardiac nerve. This should come as no surprise because in the embryo, the heart is originally located in the cervical region and only later migrates into the thorax followed by its own innervation.
The latter is bounded by the lower border of the omohyoid muscle, by the posterior border of the sternocleidomastoid muscle and by the posterior edge of the clavicle. This second triangle is much more important because it corresponds to the neurovascular bundle of the supraclavicular fossa (brachial plexus, subclavian artery). Occasionally, the omoclavicular triangle proves to be virtual, as the omohyoid muscle is partly hidden behind the clavicle (Fig. 3). In this case, the muscle needs to be removed if the anatomical pathologist wishes to expose the neurovascular bundle or displaced by the surgeon attempting to reach the subclavian artery.
